This shot is linked to an Achievement called “Die Nussknacker,” but to complete that, you will have to jump through a few more hoops.

Sniper Elite 5: How to get a testicle shot
Each hit on an enemy in Sniper Elite 5 will be mentioned as to which body part you got, alongside the various bonuses you will get. Therefore, it is no surprise that hitting a testicle will be worth quite a lot in terms of skill shots.
Players have to make sure they are only using a sniper for this shot, given how other rifles and secondary weapons will not be able to achieve the desired goal.
You will have to attempt it from some distance out, with the full lower body visible to give the best chance. Aim for the balls, and make sure the scope is focused a little lower than usual because if you put it too high, the strike will count as a hit on the body.
Since the target area is smaller than the usual parts you strike, it may take a few hits to get the right shot. However, once you achieve the testicle shot, the game will let you know as all the times before.

Completing the Die Nussknacker achievement
The game has differentiated between the regular testicle shot and finishing Die Nussnacker. The main area of difference is the distance from which you have to take the shot.
Die Nussnacker is completed when you complete a testicle shot from 100 meters or more. Therefore, you have to be extremely aware of your aim and the blowing winds, because they will come into play.
You can achieve this trophy anywhere across the game, with one of the best places being during the opening part of the ‘Spy Academy.’
This is the third Mission of the game where you will start on a beachside, and make your way to the first waypoint. Before reaching that, you will encounter a lot of enemies so take your pick in achieving Die Nussknacker will be easier.
